What ECM Referrals Offer
Personalized Care Coordination:
- Help making and keeping medical appointments
- Support with prenatal and postpartum care
- Connection to pediatric care for your baby
Mental Health & Emotional Support:
- Referrals to therapists or counselors
- Support managing stress, anxiety, or depression during and after pregnancy
Access to Social Services:
- Help with applications for housing or shelter
- Access to food programs like WIC or food banks
- Transportation support for medical or essential appointments
Advocacy and Ongoing Support:
- A dedicated care team that checks in regularly
- Help understanding Medi-Cal benefits
- Assistance communicating with doctors and service providers
